Revive MD offers optional text message (SMS) communications to improve your patient experience. By providing your mobile phone number and consenting to receive text messages, you agree to the terms outlined below.

- Patient portal notifications

SMS messages are intended to improve communication and convenience but are not a substitute for medical advice.

Standard message and data rates from your mobile carrier may apply.
Revive MD is not responsible for any charges imposed by your wireless service provider.

To stop receiving text messages:
- Reply STOP to any SMS message.
- Contact our office and request removal from SMS communications.
After opting out, you may still receive limited administrative communications when required by law or necessary for the management of your account.
Privacy and Security
While Revive MD uses reasonable safeguards to protect your personal information, text messaging is not always a secure method of communication.
For your protection:
- Do not send sensitive medical information by text message.
- Do not send credit card or payment information via SMS.

Medical Emergencies
SMS messaging is not monitored continuously and should never be used for medical emergencies.
If you are experiencing a medical emergency:
- Call 911 immediately.
- Go to your nearest emergency department.
Do not rely on text messaging for urgent medical concerns.
Changes to SMS Communications
Revive MD reserves the right to modify or discontinue SMS communications at any time.
We may update this Consent periodically. Continued participation in SMS communications after updates are posted constitutes acceptance of the revised terms.
